ExeVir announces first subjects dosed Phase 1 clinical study of potent COVID-19 neutralizing antibody

Belgium, 18 August 2021: ExeVir, which is developing single domain antibody therapies providing broad protection against viral infections, today announces that the first subjects have been dosed in a Phase I clinical study of XVR011, its llama-derived antibody for the treatment and prevention of COVID-19.

The randomized, double-blinded, single-center, placebo-controlled Phase I clinical study will evaluate the safety profile and pharmacokinetics of XVR011 administered as an IV infusion in healthy subjects. The study will sequentially test three ascending doses of intravenously administered XVR011 in a maximum of three groups of 10 healthy adult subjects.

Importantly, XVR011 was recently demonstrated to neutralise the Delta variant, as well as all current COVID-19 variants of concern. ExeVir recently announced that novel data generated in the laboratory of Professor Johan Neyts at the Rega institute (KU Leuven, Belgium) show that XVR011 demonstrated strong in vitro neutralization potency against the Variants of Concern Delta (B.1.617.2) and Gamma (P.1). Additionally, results from Professors Xavier Saelens and Nico Callewaert based on epitope sequencing indicate that the potency is not expected to be impacted by any currently circulating variant of concern or variant of interest. This is of particular relevance for the further clinical development and broad applicability.

The XVR011 molecule was developed by VIB-Ghent University (Belgium) scientists led by Professors Xavier Saelens and Nico Callewaert. The Belgian-based global biopharmaceutical company UCB helped design and optimize the therapeutic properties of XVR011 and manufactured the antibody at large scale for the clinical trial. The development work is presently undergoing peer review and was recently pre-printed in BioRxiv, demonstrating highly potent and broad neutralizing activity and infection protection in both hamster and mouse models against SARS-CoV-2.

Belgique, le 18 août 2021 : ExeVir, qui développe des traitements par anticorps à domaine unique offrant une large protection contre des infections virales, annonce aujourd’hui que le premier sujet a reçu une dose de XVR-011, l’anticorps dérivé du lama qu’elle développe pour le traitement et la prévention des infections de COVID-19, dans le cadre d’une étude clinique de phase 1.

Cette étude clinique de phase I randomisée, en double aveugle, monocentrique et contrôlée par placebo doit évaluer le profil de sécurité et la pharmacocinétique du XVR011 administré par voie intraveineuse chez des sujets sains. L’étude évaluera de manière séquentielle trois doses progressives de XVR-011 administrées par intraveineuse sur 3 groupes maximum de 10 sujets adultes sains.

ExeVir a récemment annoncé de nouvelles données obtenues par le laboratoire du professeur Johan Neyts à l’institut Rega (Université catholique de Louvain, Belgique). Celles-ci démontrent une forte capacité de neutralisation in vitro de XVR011 au niveau des variants préoccupants delta (B.1.617.2) et gamma (P.1). En outre, les professeurs Xavier Saelens et Nico Callewaert ont obtenu de résultats par séquençage d’épitopes indiquant que cette capacité ne devrait être affectée par aucun des variants préoccupants actuels ou à venir, actuellement en circulation.

La molécule du XVR-011 a été développée par des chercheurs du VIB de l’Université de Gand, pilotés par les professeurs Xavier Saelens et Nico Callewaert. La société biopharmaceutique internationale basée en Belgique, UCB, a participé à la conception et à l’optimisation des propriétés thérapeutiques du XVR-011, et a fabriqué à grande échelle l’anticorps destiné à l’essai clinique. Le travail de développement est à présent en phase de revue par les pairs et a récemment été prépublié dans BioRxiv, démontrant une large et très puissante activité de neutralisation et de protection contre les infections de type SARS-Cov-2, à la fois sur des modèles de hamsters et de souris. Le XVR-011 offre également une capacité de neutralisation équivalente des variants préoccupants alpha (B.1.1.7) et bêta (B.1.351) et se lie au domaine de liaison des récepteurs de la protéine Spike d’un spectre exceptionnellement large de coronavirus apparentés.

  • Les anticorps à domaine unique dérivés du lama sont beaucoup plus petits que les anticorps humains et peuvent se fixer sur les virus à des emplacements qui seraient difficiles d’accès pour le système immunitaire de l’homme.
  • XVR011 inactive les protéines de spicule du SARS-CoV-2 et bloque de manière stérique les spicules se liant à l’ACE2 empêchant le virus de pénétrer dans la cellule humaine et stoppant la réplication virale.
  • XVR011 complète la réponse immunitaire du patient à un moment crucial, durant lequel le système immunitaire de nombreux patients réagit trop lentement ou de manière inadaptée, lui accordant plus de temps pour remplir sa fonction et éliminer le virus.
  • L’épitope de XVR011 est bien moins sensible à la pression immunitaire des anticorps humains, susceptible d’entraîner un « échappement viral », ce qui permet de conserver une certaine puissance contre ces variants d’échappement. Aucun impact des variants concernés sur la puissance de XVR011 jusqu’ici.

XVR011 : cible l’épitope unique dans la région conservée, ce qui entraîne un large spectre de liaison aux RBD de spicules sur de nombreux sarbecovirus.

België, 18 augustus 2021: ExeVir, dat behandelingen ontwikkelt met enkeldomeinantlichamen die een brede bescherming bieden tegen virale infecties, kondigt vandaag de dosering aan van de eerste proefpersoon in een fase I klinische studie van XVR-011, zijn uit de lama afgeleide antilichaam voor de behandeling en preventie van COVID-19.

De gerandomiseerde, dubbelblinde, single-center, placebogecontroleerde fase I klinische studie zal het veiligheidsprofiel en de farmacokinetiek beoordelen van XVR-011, toegediend als IV-infuus bij gezonde proefpersonen. De studie zal drie opeenvolgende toenemende dosissen van intraveneus toegediend XVR-011 testen, met maximaal 3 groepen van 10 gezonde volwassen proefpersonen.

Na de oprichting van ExeVir 1 jaar geleden zijn wij bijzonder verheugd dat ons hoofdproduct XVR-011, nu in de fase van de klinische ontwikkeling verkeert.  Belangrijk is dat onlangs is aangetoond dat XVR011 de Delta-variant neutraliseert, evenals alle huidige COVID-19-varianten die zorgen baren. Johan Neyts van het Rega Instituut (KU Leuven, België) aantonen dat XVR-011 een sterk neutralisatievermogen heeft in vitro tegen de zorgwekkende varianten delta (B.1.617.2) en gamma (P.1). Bovendien wijzen de resultaten van dr. Xavier Saelens en dr. Nico Callewaert  op basis van epitoopsequentiëring er op dat het vermogen niet zou worden aangetast voor om het even welke van de zorgwekkende of belangwekkende varianten die momenteel in omloop zijn. Dit is van bijzonder belang voor de verdere klinische ontwikkeling en brede toepasbaarheid.

De XVR-011-molecule werd ontwikkeld door wetenschappers van VIB-Universiteit Gent onder leiding van dr. Xavier Salens en dr. Nico Callewaert. Het in België gebaseerde biofarmaceutische bedrijf UCB leverde bijstand voor het ontwerp en de optimalisatie van de therapeutische eigenschappen van XVR-011 en produceerde het antilichaam op grote schaal voor de klinische proef. Het ontwikkelingswerk ondergaat nu een peer review en werd onlangs als pre-print gepubliceerd in BioRxiv. In zowel hamster- als muismodellen toont het een uiterst krachtige en brede neutraliserende werking en bescherming tegen infectie met SARS-CoV-2.

XVR011, de belangrijkste ontwikkeling van ExeVir, is een enkeldomein anti-SARS-CoV-2 antilichaam (VHH-Fc), geoptimaliseerd voor stabiliteit, veiligheid, breed neutraliserend vermogen en uitstekende produceerbaarheid. Het heeft het beste potentieel in zijn klasse, met een breed spectrum en een krachtige werking tegen een reeks coronavirussen, en onderscheidt zich significant van andere antilichaambehandelingen

  • Het enkeldomein-antilichaam (VHH) anticoronavirusplatform werd ontwikkeld door professoren Xavier Saelens en Nico Callewaert, twee wetenschappers van VIB en UGent
  • De uit de lama afgeleide enkeldomein-antilichamen zijn kleiner dan menselijke antilichamen en kunnen zich hechten aan delen van een virus die moeilijk toegankelijk zijn voor het menselijk immuunsysteem
  • De inactiveren van de spike-eiwitten van SARS-CoV-2 en de sterische blokkering van de spike-binding met ACE2 door XVR011 beletten dat het virus een menselijke cel binnendringt en stoppen de replicatie van het virus. Dit kan de eigen immuunrespons van de patiënt aanvullen in het kritieke tijdvenster waarin het immuunsysteem van veel patiënten te traag of ontoereikend reageert, zodat het immuunsysteem meer tijd krijgt om zijn werk te doen en het virus te elimineren
  • Het epitoop van XVR011 is veel minder ontvankelijk voor immuun druk van menselijke antilichamen die tot “virale ontsnapping” kan leiden, zodat het werkzaam blijft tegen dergelijke ontsnapte varianten – Tot op heden geen impact van de zorgwekkende virusvarianten op de werkingskracht van XVR011
  • XVR011: Richt zich op een uniek epitoop in een geconserveerd gebied in het receptorbindend domein van het spike-eiwit van een groot aantal sarbecovirussen
